Meegeven lokaal anestheticum

Meegeven van een lokaal anestheticum voorkomt dat de patiënt met een keratoconjunctivitis photoelectrica de huisarts na korte tijd opnieuw consulteert. In een groot deel van de

oogheelkundige literatuur wordt gewaarschuwd voor het meegeven van anesthetische oogdruppels, omdat ze cornea-ulcera kunnen veroorzaken die uiteindelijk leiden tot blijvende visusdaling. Een en ander is vermoedelijk gebaseerd op casuïstiek uit de jaren zeventig. De

betrokken patiënten gebruikten de anesthetische oogdruppels gedurende langere tijd (vaak enkele weken) en kwamen in de vicieuze cirkel van de zogeheten anaesthesia dolorosa terecht, waarbij de cornea uiterst pijnlijk is, ondanks het feit dat hij ongevoelig is voor aanraking. 112 Niet geheel duidelijk is of de ulcera ontstaan als gevolg van door de anesthesie onopgemerkte uitbreiding c.q. infectie van een reeds aanwezige erosie, van een traumatische beschadiging door veelvuldig wrijven in het aangedane oog of van een direct toxisch effect van het anestheticum op de cornea. 113 114 115 Met de in de standaard genoemde voorzorgen (beperking van de mee te geven hoeveelheid en toepassingsduur en de instructie niet te wrijven) lijkt de kans op het ontstaan van ulcera vrijwel nihil.
