U word genooi om deel te neem aan ’n navorsingsprojek. Lees asseblief hierdie inligtingsblad op u tyd deur aangesien die detail van die navorsingsprojek daarin verduidelik word. Indien daar enige deel van die navorsingsprojek is wat u nie ten volle verstaan nie, is u welkom om die navorser daaroor uit te vra. Dit is baie belangrik dat u ten volle moet verstaan wat die navorsingsprojek behels en hoe u daarby betrokke kan wees. U deelname is volkome vrywillig en dit staan u vry om deelname te weier. U sal op geen wyse hoegenaamd negatief beïnvloed word indien u sou weier om deel te neem nie. U mag ook ter eniger tyd aan die navorsingsprojek onttrek, sonder die nodigheid om ‘n rede te verskaf, selfs al het u ingestem om deel te neem. Hierdie navorsingsprojek is deur die Komitee vir Mensnavorsing van die Universiteit Stellenbosch goedgekeur en sal uitgevoer word volgens die etiese riglyne en beginsels van die Internasionale Verklaring van Helsinki en die Etiese Riglyne vir Navorsing van die Mediese Navorsingsraad (MNR).

Wat behels hierdie navorsingsprojek?

Die deelnemers is ouers van gehoorgstremde kinders wat ten tye van dataversameling tussen die ouderdomme van 0 en 3 jaar en 11 maande oud is, en in ‘n voorskoolse klas by die Carel du Toit Sentrum is. Verdere deelnemers aan die studie sluit ook in onderwyseresse, spraak-taalterapeute en oudioloë wat betrokke is by die betrokke kinders met gehoorverlies se intervensie.

Die navorsingsprojek fokus op die persepsies van kinders met gehoorverlies se ouers, onderwyseresse, spraak- taalterapeute en oudioloë oor die uitkomste van die ouditief-verbale benadering tot opvoeding, wat hoofsaaklik gebruik word by die Carel du Toit Sentrum. Die studie het ten doel om te bepaal wat ouers, onderwyseresse, spraak-taalterapeute en oudioloë se kennis rondom die benadering is, en of hulle ten volle ingelig is oor die belangrike rol wat hulle speel in die toepassing van die benadering. Daar sal ook gepoog word om vas te stel of deelnemers se verwagtinge rondom die benadering ooreenstem met die voorgestelde beginsels en uitkomste van die benadering.

Prosedures wat gebruik sal word om data te bekom sal bestaan uit individuele semi-gestruktureerde onderhoude wat op ‘n digitale stemopnemer opgeneem sal word.

Waarom is u genooi om deel te neem?

U is genooi om deel te neem aan die studie aangesien u deel uitmaak van die teikenpopulasie, naamlik dat u ‘n spraak-taalterapeut en/of oudioloog is wat intervensie verskaf aan die betrokke kinders met gehoorverlies tussen die ouderdom van 0 en 3 jaar en 11 maande wat in ‘n voorskoolse klas by die Carel du Toit sentrum is.

146 Wat sal u verantwoordelikhede wees?

Sou u instem om deel te neem aan die studie sal daar van u verwag word om die hierdie toestemmingsvorm te onderteken en om beskikbaar te wees vir ‘n onderhoud. Elke onderhoud sal ongeveer 45 minute tot ‘n uur duur.

Sal u voordeel trek deur deel te neem aan hierdie navorsingsprojek?

U sal geen persoonlike voordeel trek uit die navorsingsprojek nie. U sal wel die geleentheid hê om vrylik u opinie te lig oor die betrokke onderwerp, en sodoende ‘n bydrae te lewer tot die kennisbasis van belang vir ouers, onderwyseresse, spraak-taalterapeute en oudioloë in die toepassing van die ouditief-verbale benadering tot opvoeding van kinders met gehoorverlies.

U sal ook na afloop van die studie skriftelike / mondelinge terugvoer ontvang oor die resultate en bevindinge. Is daar enige risiko's verbonde aan u deelname aan hierdie navorsingsprojek?

Geen identifiseerbare risiko’s is verbonde aan die navorsingsprojek nie. U identiteit sal ten alle tye beskerm word deur gebruik te maak van ’n koderingstelsel (in plaas van name en vanne) in die transkripsie, ontleding en rapportering van die data en resultate. Alle stemopnames sal na afloop van die studie vernietig word.

Sal u betaal word vir deelname aan die navorsingsprojek en is daar enige koste verbonde aan deelname? U sal nie betaal word vir u deelname aan die navorsingsprojek nie. Daar is ook geen onkoste verbonde aan u deelname aan die studie nie.

Is daar enigiets anders wat u moet weet of doen?

 U kan die Komitee vir Mensnavorsing kontak by 021-938 9207 indien u enige bekommernis of klagte het wat nie bevredigend deur die studieleier hanteer is nie.

You are being invited to take part in a research project. Please take some time to read the information presented here, which will explain the details of this project. Please ask the researcher about any questions about any part of this project that you do not fully understand. It is very important that you clearly understand what this research entails and how you could be involved. Also, your participation is entirely voluntary and you are free to decline to participate. If you do not want to participate, this will not affect you negatively in any way. You are also free to withdraw from the study at any point, even if you do agree to take part.

This study has been approved by the Committee for Human Research at Stellenbosch University and will be conducted according to the ethical guidelines and principles of the international Declaration of Helsinki, South African Guidelines for Good Clinical Practice and the Medical Research Council (MRC) Ethical Guidelines for Research.

What is this research study all about?

The participants are the parents of hearing impaired children that are between the ages of 0 and 3 years and 11 months at the time of data collection and is in a pre-school class at the Carel du Toit Centre. Other participants of the study includes teachers, speech-language therapists and audiologists that is involved in the intervention of the hearing impaired children.

The research project focusses on the perceptions of the parents of hearing impaired children, teachers, speech- language therapists and audiologists on the outcomes of a auditory-verbal approach, when used in the education, that is mainly used at the Carel du Toit Centre. The study aims to determine the knowledge of parents, teachers speech-language therapists and audiologists regarding the approach, and to determine if they are fully informed about the part they play in the application of the approach. The study will also try to determine if participants’ expectations regarding the approach corrolates with the proposed principles and outcomes of the approach.

Procedures that will be used to collect data will include individual semi-structured interviews that will be recorded on a digital voice recorder.

Why have you been invited to participate in this process?

You have been invited to participate in this process, as you are part of our target population, i.e., speech-

language therapists and/or audiologists that is involved in the intervention of hearing impaired children between the ages of 0 and 3 years and 11 months at the Carel du Toit Centre.

148 What will your responsibilities be?

If you agree to partake in this study it will be asked of you to sign this form of consent.

Your other responsibilities would be to be available for an interview at a scheduled time. The duration of each interview will be approximately 45 minutes.

Will you benefit from taking part in this research?

You will not receive any personal gain from the research project. You will, however, have the opportunity to state freely your opinion about the topic at hand and to contribute to the knowledge base concerning the importance for parents, teachers speech-language therapists and audiologists in the application of the auditory- verbal approach in the education of hearing impaired children. You will also receive written/verbal feedback on the results of the study after completion thereof.

Are there any risks involved in your taking part in this research?

There are no identifiable risks associated with this research project. Your identity will be protected at all times through use of a coding system (as opposed to using names or surnames) in the transcription, analysis and reporting of the results. All audio recordings will be destroyed upon completion of the study.

Will you be paid to take part in this study and are there any costs involved?

You will not be paid to take part in this study, and should you partake in the study, there will be no costs involved for you.

Is there any thing else that you should know or do?

 You can contact the Committee for Human Research at 021-938 9207 if you have any concerns or complaints that have not been adequately addressed by the investigator.
