Richtlijnen voor anti-infectieuze behandeling in ziekenhuizen BVIKM 2017
ACNE VULGARIS:
ERNSTIGE PAPULOPUSTULAIRE ACNE, MILDE OF MATIGE NODULAIRE ACNE
• Klinische aspecten en commentaren o Inflammatoire vorm van acne vulgaris.
o Aanwezigheid van comedonen, papels en pustels, kleine nodules. o Topische antibiotica: niet aanbevolen.
• Betrokken pathogenen o Propionibacterium acnes.
• Empirische anti-infectieuze behandeling o Regimes en standaard posologieën.
Eerste keuze: isotretinoïne po. Alternatieven.
Doxycycline (200 mg po q24h of 100 mg po q12h) + adapalene (0,1% gel/crème topische applicatie q24h bij het slapengaan).
Doxycycline (200 mg po q24h of 100 mg po q12h) + azelaïnezuur (20% crème topische appli-catie q12h).
Doxycycline (200 mg po q24h of 100 mg po q12h) + [adapalene (0,1% gel) + benzoyl peroxide (2,5% gel)] topische applicatie q24h bij het slapengaan.
Lymecycline (300 mg po q24h) + adapalene (0,1% gel/crème topische applicatie q24h bij het slapengaan).
Lymecycline (300 mg po q24h) + azelaïnezuur (20% crème topische applicatie q12h). Lymecycline (300 mg po q24h) + [adapalene (0,1% gel) + benzoyl peroxide (2,5% gel)]
topi-sche applicatie q24h bij het slapengaan. Te overwegen bij de vrouwelijke patiënten.
Doxycycline (200 mg po q24h of 100 mg po q12h) + een oraal anti-androgeen. Lymecycline (300 mg po q24h) + een oraal anti-androgeen.
Adapalene (0,1% gel/crème topische applicatie q24h bij het slapengaan) + een oraal anti-androgeen.
Azelaïnezuur (20% crème topische applicatie q12h) + een oraal anti-androgeen.
Benzoyl peroxide (5% of 10% gel topische applicatie q12-24h) + een oraal anti-androgeen. o Totale duur van de adequate (empirische + gedocumenteerde) anti-infectieuze behandeling.
Optimale duur is niet gedefinieerd. Antibiotica: maximum 6 maand.
