The work presented in this thesis was performed at the Department of Health Sciences, VU University Amsterdam, the Netherlands, at the Department of Biomedical Sciences, Institute of Tropical Medicine, Antwerp, Belgium, at the National Institute of Hygiene, Epidemiology and Microbiology, Havana, Cuba, and at the Pedro Kourí Institute, Havana, Cuba.
