Alhamdulillahirabbil’alamin. It is a remarkable achievement that I’ve arrived at this stage. Completing a doctoral degree, support and academic contributions of many individuals. The work described in this thesis is indebted to many people that I have worked with, collaborated with, agreed, and disagreed with over the past several years. The completion of the thesis described here would not have been possible alone and it’s important to acknowledge the assistance and support of those that have contributed to its development.

Thank you for all the prayers from my mother Ir. Ratna Dalimi and my father Ir. Affiar AS my sister dr. Ira Masykura M.Kes, my brothers Rahmadil Adha ST and Addinul Ichsan ST, MM, my mother-in-law dra. Darmawaty, my father-in-law dr. Zulkifli Djamilah Sp.PD, my sister-father-in-law Dr. Eng. Desriani, Dr. Dian Hafizah and my brother-in-law dr.Rinal Effendi Sp.An and dr. Bagus Demawan.

I would like to express my sincere thanks to you as my supervisor, Prof. Maarten J. Postma, who has given me the opportunity to do this enjoyable PhD, and my co-supervisor Dr. A.D.I van Asselt.

Dear Prof. Maarten J. Postma,

I still remember when the first time I contacted you after getting your email address from the university website. I was very shocked when you replied to my email immediately and agreed to be my supervisor. You contacted me several times only to ask about the progress of my scholarship application. It was a fast process. I could easily get my scholarship to the University of Groningen. But I have to postpone my PhD journey to the next year due to my pregnancy. I have learned many things from you to be an independent researcher. One more thing that I can not forget is you always add “notably” in every article we collaborate with. Thank you for giving me flexibility both on research ideas and working hours during my study. I really appreciate it. Again, I am grateful that you always support me both in academic and personal matters.

Dear Dr. A.D.I van Asselt,

Hai Thea, we first met actually at the University of Groningen. I still remember I lost my way several times when I have to go to your office. I always use the outside route from Pharmacy to Epidemiology in my first year of PhD. I could use the internal route in my second year of my PhD. I don’t have a good ability in memorizing maps. I got an impression that you are a very tough mom, certainly inspires other moms, like me. I never expect that you are also a singer, of course, it makes me more confident in showing my ability in the musical instrument. Thank you for visiting me in West-Sumatera. I wish we could do something fun together again.

I would like to express my appreciation to the Indonesian Government for the scholarship from the Directorate General of Higher Education (DIKTI), and Universitas Andalas which has encouraged me to keep studying despite the many obstacles that stand in my way.

Thank you my colleagues and friends in the Department of PharmacoTherapy, -Epidemiology,& -Economics for supporting me during my study. I could not have succeeded without my invaluable friends: Akbar, Ivan, Yuan, Silvy, Neily, Didik, Sofa, Christiaan, Simon, Pepijn, Renata, Eva, Qi, Wandi, Khairul, Mba Ira, Hao, Renata, Abraham, Lan, Linda, Floriaan, and Tanja. Especially my roommate: Jurjen, who will always become a “big guy” but for me, you look like my younger brother. Pieter who always sings “rasa sesal” every time you’ve got bored. Ury, who always forces me to sing every Friday afternoon after office hour. Next, my travel mate Lusiana. Dear mba Lusi, I think we are not just travel mates, but we share a philosophy of life. Trying to live a happy life with our beloved family.

For the “hore-hore” team. Ga ada kalian ga rame: Mas Lutfi, the most mature guy among us even though it ended up being childish because of us. Mba Lusi, who always gives new ideas about the new area to be visited and new places to stay that are of course cozy and cheap. Putri, I call you dek put because you are the youngest woman in this group. Pak Guru Muzakki, the guy who spends the longest time tidying up himself before leaving, but has the greatest English skills among us. Alfian, the most adorable and singing boy, I like the local dialect you use. Laras, the last one who always be a silent reader in our group. For the “partai ambuih-ambuih”. A group that is decades old (almost 20 years) but still exists in the world of pharmaceutical issues: Tila, Devi, Weli, Yandi,

Dino, Anton, Dedi, Dola, Shinta, Fauzan, Fiken, Irma, Angel, Kak An, Leni, Qoni, Rike, Ami, Toni, Tyas, Wulan, Tedy and Daeymont. There is no day without “ambuih-ambuih”. Until now I don’t know what is the right word to describe this group. Maybe someday we can change the name to “hembus-hembus” so that it feels a little mind-blowing.

For UNAND ranger in the Netherlands. Uniang, Ichsan, and Pak Koko. Gaining a degree in this country is difficult but nothing is impossible. Like what Bung Karno said: “Give me 10 youths, then I’ll shake the world” but because we are still few in number, we still can only shake just our village, while cooking, traveling, and taking photos.

Next, for all my friends and colleagues at Universitas Andalas, thank you for your support. Especially for the academic staff, thank you for helping me with my correspondence and administrative stuff that often make me feel exhausted. For the finance staff who fought for my finances when I returned to Indonesia. For all junior lecturers, keep fighting. There is still a long way to go to be able to fight for your fate.

Finally, I would like to thank you my beloved family for your wonderful support during these awesome periods.

Dear my beloved husband, Ade,

You always stand with me. Me, who is a stubborn wife. Holding your hand and breathing is a kind of relief. After long-distance marriage, we made it, we passed it. We are stronger than we ever thought possible, although it is the kind of strength that comes from weakness, failure, loss, and tears. The storms raged and I was terrified. Thank you for staying on the boat with me. I am so thankful to be your wife. I thank God for you.

Dear Abang Rakhsan,

You are growing up now, faster and more mature than I expect. Be a role model for relatives and friends. Time has taught us to process and pass the good and bad times together. I see the light in you, like your name which means light.

Dear Hikari,

You were born when daddy was doing his PhD in Japan, that’s why we named you Hikari. Like abang Rakhsan, I also see the light in my little baby who is now growing up to be a sweet and smart little girl.

Dear Ama and Apa,

Thank you for helping to look after the children when I had to struggle with my PhD. Your help and support are invaluable.
