I would like to extend my thanks to the many people, in many hospitals around the Netherlands, who so generously contributed to the work presented in this thesis. The physicians and nurses that conducted the record reviews used in this thesis deserve a special thanks for their tireless work. Special mention goes to my supervisor, Cordula Wagner. I thank Cordula, not only for her academic support and enthusiasm, but also for giving me the opportunity to grow as a researcher.

I am also hugely appreciative to Maaike Langelaan, especially for sharing her statistical expertise and practical experiences so willingly, and for being so dedicated to her role as my secondary supervisor.
