Vervolgen wondgenezing

De multidisciplinaire richtlijn adviseert de wondgenezing te volgen met een gevalideerd meetinstrument, zoals de Pressure Ulcer Scale for Healing of de Decubitus Wond Score. 1 Het eerstgenoemde instrument is door de NPUAP ontwikkeld, waarbij op basis van bestaand materiaal gekeken is naar de factoren die wondgenezing het best voorspellen. Drie factoren (lengte en breedte, hoeveelheid exsudaat en dominante weefselsoort) verklaarden 55 tot 65% van de variantie van de wondgenezing en zijn om die reden opgenomen in de Pressure Ulcer Scale for

Healing. 97 Het instrument is in diverse onderzoeken gebruikt om genezing te vervolgen. 9598 Omdat het instrument weinig anders is dan de optelsom van de gegevens wondgrootte, dominante weefselsoort (zwart, geel of rood) en een schatting van de hoeveelheid exsudaat, is besloten de genezingstendens van de wond wel aan de hand van deze kenmerken te beoordelen, maar het instrument als zodanig niet te gebruiken.

De Decubitus Wond Score is een veel uitgebreider beoordelingsinstrument, waarin ook diverse andere variabelen die iets zeggen over de toestand van de wond zijn meegenomen, zoals mate van ondermijning en kleur van de huid of oedeem rondom de wond. 100 Het is niet duidelijk of het instrument afdoende gevalideerd is. Het instrument lijkt daarnaast te complex voor gebruik in de huisartsenpraktijk.
