Introductie:

-‐ Wij zijn Jochem, Laura and Carlien, Master studenten Neurobiologie die momenteel de Tesla
Minor doen. De tesla minor is bedoeld voor Beta studenten die meer concreet hun kennis willen gebruiken zonder wetenschappelijk onderzoek te doen. We werken momenteel aan een project voor TNO genaamd Pharma Nutrition, waarin we de mogelijkheden onderzoeken om voeding en
medicijnen optimaal te laten werken door er een combinatie van te maken voor Type 2 Diabetes
-‐ Het doel van het project is een showcase te maken waarin we laten zien dat pharma nutrition
kan werken bij het (kosten)effectiever maken van de behandeling van type 2 diabetes.
-‐ Uw informatie zou ons kunnen helpen meer inzicht te krijgen in de huidige behandeling van type
2 diabetes. Ook zou u kunnen vertellen waar u tegen aan loopt als zijnde problemen in de praktijk. In hoeverre kan voeding effectief zijn in de behandeling van T2D en waar liggen de
verbeterpunten.
-‐ Het interview zal ongeveer 45 minuten tot maximaal een uur duren. Als er vragen zijn die u niet
wil beantwoorden is dat goed. Ook kunt u stoppen met het interview op elk moment.
-‐ Als u het goed vind zouden we het interview graag willen opnemen en aantekeningen willen
maken om de informatie te kunnen gebruiken voor analyse. De opnames en aantekeningen
zullen verwijderd worden zodra de analyse gedaan is.

IABETESThe disease
Type 2 Diabetes (T2D) is a metabolic disorder that is associated with decreased levels of insulin, resulting in elevated blood sugar levels. Elevated blood sugar levels, or hyperglycemia, are associated with micro-‐ and macrovascular problems (Stratton, Vella, Sheeshka, & Duncan, 2015). Besides glucose and blood sugar levels, T2D is also associated with oxidative stress and disturbances in the balance of lipid levels (Donath & Shoelson, 2011; Giacco & Brownlee, 2010; Natarajan & Nadler, 2004; Rains & Jain, 2011). Other big risk factors for vascular problems are oxidative stress and lipid metabolism. These vascular complications range from retinopathy (eye problems), amputation, and neuropathy, to stroke and heart attacks that can potentially lead to death. These severe consequences underline the importance to not only manage glycemic levels, but also other pathological responses such as oxidative stress and lipid metabolism (Giacco & Brownlee, 2010; Natarajan & Nadler, 2004; Stratton et al., 2015). The main mechanism of disease of T2D is the production and uptake of insulin. Insulin is released in the pancreas and is the main controlling factor of glycemic levels because it increases the uptake of glucose by fat and muscle tissue, and decreases the production of glucose in the liver (Kahn, 1998). With the progression of T2D, the production of insulin is gradually negatively affected. Early In the disease progression, the sensitivity to insulin is reduced. At first, the pancreas is able to compensate this disturbance by releasing increased amounts of insulin. However, as T2D progresses, this decreased sensitivity continues, and the pancreas will not be able to keep up and will eventually shut down. The moment when the initial disbalance in insulin production turns into the disease T2D is when the sober blood glucose levels rise above a certain threshold (7,0 mmol/l over longer period of time (Rutten et al., 2013).
Insulin resistance is due to an accumulation of fatty acids in muscle and liver tissue, resulting in inhibition of insulin stimulated glucose transport (Lowell & Shulman, 2005). The reduced insulin production in the pancreas is a result of lessened function of β-‐cells (Rhodes, 2011). In addition to the
insulin metabolism being disrupted, the lipid metabolism is unbalanced which can cause cardiovascular
complications such as aterosclerose (Brault, Ray, Gomez, Mantzoros, & Daskalopoulou, 2014; Davignon,
2004).
The medicine
Usually, the first line of treatment of T2D in the Netherlands is provided by the general practitioner (GP). The treatment provided by the GP consists of multiple phases as set by the guidelines of the Nederlandse Huisartsen Genootschap (NHG) (Rutten et al., 2013). According to these guidelines, the first stage always consists of healthy lifestyle advice including physical activity and a healthy diet. If this fails to lower the blood glucose level, the first line of treatment is the oral medication Metformin. The dosage of Metformin starts at 1x per day 500 mg and can be increased to a maximum of 3x per day 1000mg.Subsequently, if this fails to function or the disease progresses to a point where it is no longer sufficient, gliclazide (a sulfonylureum derivative) is added to the treatment.
