Een proefschrift schrijf je niet alleen. Op deze plaats wil ik iedereen bedanken die op welke

manier dan ook heeft bijgedragen aan de totstandkoming van dit proefschrift. In het

bijzonder wil ik alle patiënten, en veelal ook hun partners en familieleden, bedanken die vele

vragenlijsten hebben ingevuld en hebben meegewerkt aan meerdere en vaak lange

interviews. Ondanks de moeilijke periode in hun leven waren zij bereid om mee te doen aan

wetenschappelijk onderzoek en openhartig te vertellen over hun ervaringen.
