ATC-referentiebestand FKG's psychische aandoeningen vereveningsmodel 2018
TOELICHTING1. Bij de bepaling van FKG klassen zijn de volgende farmaceutische middelen uitgesloten en niet in de FKG DDD-uitvoeringstabel opgenomen:
(a) middelen die in de G-standaard van Z-Index zijn aangemerkt als niet voor vergoeding in aanmerking komend op grond van artikel 2.8 Besluit zorgverzekering; (b) middelen die in de G-standaard van Z-Index als productgroep zijn aangemerkt als grond- en hulpstoffen.
2. Zorginstituut Nederland hanteert een drempel van meer dan 180 standaarddagdoseringen per klasse van het vereveningscriterium FKG’s psychische aandoeningen. Onder deze drempel deelt het Zorginstituut een verzekerde niet in bij een klasse van het vereveningscriterium FKG’s, anders dan ‘geen FKG’.
3. In afwijking van punt 2. hanteert het Zorginstituut voor de FKG-klasse 'Psychose depot' een drempel van één receptregel én een DDD-factor van ten minste 3500.
4. In afwijking van punt 2. hanteert het Zorginstituut voor de FKG-klassen 'Bipolaire stoornis complex' een drempel van meer dan 180 standaard-dagdoseringen voor de FKG-klasse ‘Bipolaire stoornis regulier’ én ten minste één voorschrift van de indicator 'Bipolaire stoornis complex'.
5. Zorginstituut Nederland wijst alle toepasselijke klassen van het vereveningscriterium FKG's psychische aandoeningen toe met inachtneming van de volgende uitzonderingen:
(a) Een verzekerde ingedeeld bij de FKG-klasse ’Psychose depot' deelt het Zorginstituut niet in bij de FKG-klasse ‘Psychose’;
(b) Een verzekerde ingedeeld bij de FKG-klasse ‘Bipolaire stoornis complex’ deelt het Zorginstituut niet in bij de FKG-klasse ‘Bipolaire stoornis regulier’. 6. Bij de bepaling van FKG klassen zijn de volgende farmaceutische middelen uitgesloten:
(a) middelen met werkzame stof bupropion (ATC-code N06AX12) die worden ingezet bij stoppen met roken, bij de FKG 'Chronische stemmingsstoornissen'.
