A
Description and BPMN diagrams of the FACT care processes
A.1
Admission
The process starts when the application form of the referrer is received by the central application department
or secretary of the mental healthcare institution. Some institutions also allow self-application when the
patient has a valid referral letter. The application form is received, assessed and processed by a central
application department or secretary within the mental healthcare institution, which are not part of the
FACT-team. First, the central administration department or secretary will assess the application form for
being complete. When an application form is incomplete, they will contact the GP or the patient for more
information. Based on the available information, it will be decided whether the patient is eligible for FACT.
When a crisis is expected based on the application, the patient will be referred to the crisis service of the
mental healthcare institution. When the FACT-team cannot deliver the right care to the patient, they will
refer the patient back to the referrer with advice for another treatment. Some mental healthcare institutions
will inform a patient about how to meet the application criteria when they do not comply.
The patient will be admitted to the FACT-team when the application meet the admission criteria. First, a
main care provider (Dutch: “regiebehandelaar”) from the FACT-team will be assigned to the patient who
best suits the problems and care needs of the patient. A main care provider can be a psychiatrist, clinical
(neurological) psychologist, psychotherapist, health psychologist, nurse practitioner, or addiction expert.
Then the patient and close relatives will be invited for a first intake interview. Furthermore, the central
application department or secretary will add the patient to the EHR and will register the patient details and
referral letter. They are point of contact between the referral and intake. The intake process will start when
the intake interview will take place.
A.2
Intake
The goal of the intake process is to determine the patient’s diagnosis and a preliminary treatment plan. The
duration of the intake process varied between organisations from one day to multiple weeks. Furthermore,
in some mental healthcare organisations not always a FACT-member is involved during the intake. Although
differences in duration were present, the same sub processes were found.
One or multiple FACT-members will perform the first intake interview. From then on, the main care provider
is the point of contact for the patient, close relatives, and others. The goal of the first intake interview is to
determine a diagnosis. The patient will complete a ROM questionnaire, prior or during the intake interview.
Additional examinations may take place when the situation is not clear after the first intake interview. These
examinations may include a psychological examination, psychiatric examinations, laboratory research, or
physical examination (somatic screening).
When the patient’s situation is clear, the main care provider will determine the patient’s diagnosis and
care needs and may discuss the results in a multidisciplinary meeting. It will be determined whether the
FACT-team can deliver the right care based on the diagnosis and first care needs. This information will be
recorded in an intake report which also includes a treatment advice.
Then the patient and close relatives of the patient will be invited for an advisory interview. In this interview,
the results of the intake will be discussed. The patient will get to know if the FACT-team can provide the
right care. When this is not the case, the main care provider will provide an explanation and will inform the
patient about being referred back to the GP or other referrer. They will also receive the intake report which
includes information about the diagnosis and recommendations for further treatment. When the patient
is eligible for FACT, the patient will receive information about treatment options and a treatment advice.
Together with the patient and close relatives of the patient, a preliminary treatment plan will be determined.
In this plan also agreements are included about when the treatment will start. In some cases, the main
care provider will discuss the preliminary treatment plan within the FACT-team. Also the patient will be
asked to agree with the treatment plan.
At the end of the intake process, the GP or other referrer will be informed about the conclusion, diagnosis
and proposed treatment. Furthermore, an initial Diagnosis Treatment Combination (Dutch: Diagnose
Behandel Combinatie, DBC) will be opened for the patient and care will be allocated. The preliminary
treatment plan and the intake report will be added to the EHR. The patient may receive a copy of the
treatment plan.

Termination
During the evaluation, the FACT-team may have concluded that care can be terminated. The process of
termination will be initiated when the patient meets six termination criteria for at least two years. These
criteria are related to the frequency of contact, medication use, support system, work or daytime activities,
accommodation, and financial situation. Furthermore, termination should be wished by the patient. The
case manager will make agreements with the patient and GP about the continuation of care at the GP.
Furthermore a final ROM questionnaire will be completed. Agreements will be made appointments with the
GP about returning the patient to the FACT-team when the patient shows signs of relapse or has greater
care needs after termination. Often it will be agreed that a patient can receive intensive team care from the
FACT-team without a waiting list and without the intake procedure.
The case manager will write a termination letter which contains information about that care will be
terminated, the results of the treatment and agreements which are made about the continuation of care.
With this termination letter, the care will be transferred to the GP.
After agreements are made about the continuation of care, the termination letter will be registered in the
EHR. Then, the DBC will be discontinued.

Onderzoek naar de werkwijze en behoeften van FACT-medewerkers en tevredenheid
over het EPD
Een EPD is een softwaretoepassing waarin medische patiëntengegevens in digitale vorm kunnen
worden vastgelegd door verschillende behandelaren in één dossier. Met een EPD kunnen zorgverleners
binnen één organisatie elkaars gegevens over een patiënt digitaal inzien. Voorbeelden van veel
gebruikte EPD's in de GGZ zijn: mijnQuarant, USER, NEXUS/EPD en EZIS.
