Doel van dit interview: In dit interview wil ik graag informatie over hoe de patiëntenstroom verloopt
vanuit het ziekenhuis, naar het revalidatiecentrum (begint bij het ontslaggerechtigd verklaren in het
ziekenhuis, tot aan de opname van de patiënt op de revalidatie afdeling). Daarnaast ben ik op zoek
naar mogelijke barrières die een goede patiëntenstroom tegenhouden en op zoek naar suggesties hoe

B De patiëntenstroom
Ik wil graag inzicht in hoe de patiëntenstroom tussen het ziekenhuis en het revalidatiecentrum
beginnend bij het ontslaggerechtigd verklaren in het ziekenhuis, tot aan de opname van de patiënt op
de revalidatie afdeling. Voor deze vraag maak ik gebruik van een algemene stroombeschrijving, die ik
heb verkregen door informatie samen te voegen uit observaties en gesprekken met betrokkenen uit de
patiëntenstroom. Ik heb dit op papier gezet in diagram 1. Graag zou ik van u willen weten of deze
beschrijving klopt en compleet is.

D Variatie in de patiëntenstroom
D1: De patiëntenstroom kan gezien worden als een stroom waarbinnen er variatie is als het gaat
om verschillende zorgwensen/ zorgvragen / zorgbehoeften. Een CVA-patiënt heeft over het algemeen
een complexere zorgvraag dan een patiënt die revalideert van een knieoperatie. Hoe variabel is deze
zorgvraag? Hoe gaat u om met een verschillende complexiteit aan zorgvragen in de patiëntenstroom?
Hoe gaat de organisatie om met verschillende zorgwensen? Hoe probeert u de verschillende
zorgwensen zo goed mogelijk te behandelen?
Hoe groot is de invloed van deze variatie op hoe goed de patiëntenstroom verloopt?
Wat zou beter kunnen als het gaat om het inspelen op verschillen in de zorgvraag?
D2: Verder kan er ook variatie zijn in de toestroom van het aantal patiënten. Bijvoorbeeld, de ene
dag kunnen er veel meer patiënten zorg vragen dan de andere dag. Of het ene uur kunnen er zes
aanvragen binnenkomen, terwijl het volgende uur er niets binnenkomt. Hoeveel variatie zit er in de
toestroom van het aantal patiënten wat betreft het volume? Hoe gaat u om met een variatie in de
toestroom van het aantal patienten? Hoe gaat de organisatie hiermee om? Wat voor maatregelen
worden voor deze variatie genomen om het voorspelbaarder te maken?
Hoe groot is de invloed van deze variatie op hoe goed de patiëntenstroom verloopt?
Wat zou beter kunnen als het gaat om het inspelen op verschillen in de toestroom van patiënten qua
volume?

D3: Daarnaast kan er ook variatie zijn in de behandeltijd van behandelaren. Bijvoorbeeld, de
zorgverlening kan verschillen als het gaat om snelheid van handelen. Hoeveel variatie is er in de
behandeltijden tussen behandelaren? Wat doet u om deze variatie in behandeltijden zo goed mogelijk
op elkaar af te stemmen? Wat doet de organisatie hieraan en hoe probeert de organisatie hierop in te
spelen? Wat doen de verschillende partijen in de patiëntenstroom hieraan?
Hoe groot is de invloed van deze variatie op hoe goed de patiëntenstroom verloopt?

D4: Als laatste kan er variatie in de patiëntenstroom zijn door activiteiten die geen waarde
toevoegen, bijvoorbeeld door taken die dubbel gedaan worden, te veel / te weinig ingepland personeel,
onnodige loopafstanden. Wat zijn niet waarde toevoegende activiteiten binnen de patiëntenstroom?
Wat doet u om niet waarde toevoegde activiteiten te vermijden? Wat doet de organisatie hieraan en
hoe probeert de organisatie dit te verminderen? Wat doen de verschillende partijen in de
patiëntenstroom hieraan?

Wat zijn volgens u belangrijke factoren die ervoor zorgen dat de patiëntenstroom zo optimaal mogelijk
verloopt? Zou u hardop denkend deze factoren kunnen noteren van meest belangrijk naar minst

Uit de literatuur komen drie prestatie-factoren die de patiëntenstroom kunnen beïnvloeden:
• Consistentie, dit is de mate van variatie in doorlooptijd.
• Snelheid, dit is de doorlooptijd per patient van ontslaggerechtigd tot aan de opname van
Revalidatie en Behandeling
• Veiligheid, dit is het aantal x urgente complicaties dat zich voordoet in de patiëntenstroom.
Zou u na het horen van deze factoren uw lijstje nog eens willen maken en deze drie
prestatie-factoren willen toevoegen. Hoe zit het lijstje er dan uit en waarom? Probeer mij hierover hardop
denken mee te nemen.

Appendix B: Field notes
Field notes transfer department (12
thof April)
I have visited the transfer department from the end of the morning till the end of the afternoon. The
transfer nurses are working in an office at the Hospital, where four transfer nurses can work at the same
time. First, the transfer nurse showed me the internal information system that they are working with, the
Electronic Patient Nursing Record (EPNR). In this information system all information of the patient are
noted. The task of the transfer nurse is to fill in the request from of the coordinating party, which is
different from the form in the EPNR. The transfer nurse works with two screens, one screen that shows
the e-mail for new requests, communication with colleagues and communication with the patient or
family of the patient. On the other screen the EPNR and request form for rehabilitation is shown. The
transfer nurse has to switch between different screens and systems to find the needed information of the
patient, which has to be copy-paste into the request form. Sometimes, the transfer nurse indicated that
when the patient is still not admitted to the hospital, the transfer nurse needs to fill in the basic
information of the patient themselves. Besides that, they have two computer screens to work on, they
have a headset to answer phone-calls from patients or colleagues, which makes a busy impression.
During the visit, the transfer nurse had to answer the telephone several times during the performance of
her work. Furthermore, the transfer nurse received multiple e-mails during the day. The requests that are
sent to the coordinating party are sent in a secure way via e-mail with the request attached.
The transfer nurse indicated that they would like to receive every request for rehabilitation from the
nursing ward 24 hours in front, before the patient can be discharged. This is because then they strive to
fill in the form and send it to the coordinating party before the patient is discharged. Furthermore, the
transfer nurse adds that Tuesdays are the busiest days, because all the disciplines in the hospital have
multidisciplinary meetings, where the transfer nurse wants to be present.
Between calling and working on completing the requests, the transfer nurses consult each other about
how they will deal with the work of the day and discuss different cases. Furthermore, the transfer nurses
have to visit patients inside the hospital, which causes that the transfer nurse has to make trips during
the day.
Field notes coordinating party (10

I have visited the coordinating party in the morning. The coordinating party is led by nurses that do the
mediation of patients from the hospital to the rehabilitation care provider, home care or GP. What
immediately stands out are the large stacks of study on the desks. The coordinating nurse indicates that
every request is printed out and is kept on study for the administration.
The coordinating nurse shows the information / mediation system that is used and shows what the request
forms look like. During the visit, the coordinating nurse is not performing her work but explaining how
the system works. The processing of the requests is kept in the care monitor. When the request comes

in, the request is checked by one of the coordinating nurses if the information and indication that is given
is correct. Next, the coordinating nurse looks if there is a bed available at the rehabilitation care provider
according to the preference of the patient. If there is no bed available at the preference of the patient, the
patient is mediated elsewhere.
The coordinating party indicates that requests for rehabilitation are coming in on the same day that the
patient is ready for discharge, which causes that the patient in the hospital has to wait one more day to
be transferred if there is a spot. This is not only to the detriment of the patient but also of the hospital,
as an occupied bed for a day naturally brings costs.
Field notes customer service (24
thof April)
I have visited the customer service of the rehabilitation care provider in the afternoon. The customer
service is led by transfer nurses, that have the capacity to work with four nurses at the same time. The
requests for rehabilitation that have been received are printed out for assessment of the rehabilitation
specialist. The customer service receives the request via e-mail, with the handover as an attachment. The
task of the customer service is to indicate whether there are beds available in the organization, to check
the handover if everything is concluded and complete and facilitates the rehabilitation specialist in
deciding on the admission of a patient. Next to that, the customer service has to update the bed capacity
on the care monitor of the coordinating party. The customer service indicates that there are basic
inclusion or exclusion criteria for providing rehabilitation and other follow-up care. However, the
customer service indicated that there were no clear inclusion or exclusion criteria for accepting the
patient regarding rehabilitation.
The requests for rehabilitation and other follow-up care are assessed once a day, in the start of the
afternoon. If it is necessary, the rehabilitation specialist can also assess the requests later in the afternoon.
This is in contradiction to the directive of the coordinating party about responding to requests (within 4
hours). Sometimes, the rehabilitation specialist has questions about the handover, which has to be send
back to the hospital, that causes that the deadline of 4 hours cannot be made.
The planning information regarding capacity of beds at the rehabilitation departments is being monitored
in an internal excel system, which is only visible for the customer service. In this document, the
rehabilitation department, the planned dismissal date and the indication of the patient are filled in, so
that the customer service can see when beds become available again. The customer service indicates that
they are not planning the patients in front, as they sometimes not guarantee that there is a spot available
at that day, because there can always change something in the rehabilitation process.
