I n d ices of d i s c r i m i n a t i o n (i.e., the a b i l i t y to i d e n t i f y t a r g e t f r o m d i s t r a c t o r i t e m s d u r i n g r e c o g n i t i o n m e m o r y testing) a n d r e s p o n s e b i a s (i.e., t h e p r o b a b i l i t y of s a y i n g ''yes" t o an i t e m w h e n u n c e r t a i n if it is a t a r g e t or a distractor) h a v e b e e n s u g g e s t e d as u s e f u l a i d s w h e n m a k i n g su c h a d i f f e r e n t i a l d i a g n o s i s . T h i s s t u d y a t t e m p t e d to a d d r e s s t h e n a t u r e of d i s c r i m i n a t i o n a b i l i t y and r e s p o n s e b i a s in p a t i e n t s w i t h d e m e n t i a c o m p l i c a t e d by depression,, as c o m p a r e d to d e m e n t e d p a t i e n t s w i t h o u t a f f e c t i v e s y m p t o m s and d e p r e s s e d p a t i e n t s w i t h o u t c o g n i t i v e impairment. T h e d e m e n t e d / d e p r e s s e d g r o u p w e r e also m o r e c o n s e r v a t i v e t h a n t h e d e p r e s s e d group, b u t this c o m p a r i s o n d i d n o t r e a c h s t a t i s t i c a l signif i c a n c e . M a j o r d e p r e s s i o n c o m p l i c a t e s u n e q u i v o c a l d e m e n t i a in 1 0 - 2 0 % of d e m e n t e d p a t i e n t s and s i g n i f i c a n t d e p r e s s i v e

f e a t u r e s o c c u r in 40-50% of d e m e n t e d p a t i e n t s (Wragg, 1989). T h e d i a g n o s i s of d e p r e s s i o n in d e m e n t e d patie n t s , however, is o f t e n c o n f o u n d e d b y the o v e r l a p of s y m p t o m s of d e p r e s s i o n and d e m e n t i a (e.g., p s y c h o m o t o r changes, a p p e t i t e a n d s l eep d i s t u r b a n c e s , d i m i n i s h e d i n t e r e s t ) . T h e s e symptoms, c o m m o n l y a s s o c i a t e d w i t h d epression, are a l s o o f t e n p a r t of the

T h e r e f o r e , s u c c e s s f u l i d e n t i f i c a t i o n and t r e a t m e n t of d e p r e s s i o n in d e m e n t e d p a t i e n t s m a y r e p r e s e n t t h e o n l y " i m p r o v a b l e " c o m p o n e n t in an o t h e r w i s e u n r e m i t t i n g c o g n i t i v e d e t e r i o r a t i o n . T h e e s t a b l i s h m e n t of a n e u r o p s y c h o l o g i c a l t o o l to a i d t h e i d e n t i f i c a t i o n of d e p r e s s i o n in d i a g n o s t i c a l l y e n i g m a t i c d e m e n t e d p a t i e n t s w o u l d b e an i m p o r t a n t c o n t r i b u t i o n to d i f f e r e n t i a l d i a g nosis. d e p r e s s i o n and. t o a t t e m p t to i d e n t i f y d i s t i n g u i s h i n g c h a r a c t e r i s t i c s t h a t m a y a s s i s t w i t h d i f f e r e n t i a l diagnosis. S p e c i f i c a l l y , d e m e n t e d / d e p r e s s e d p a t i e n t s w e r e c o m p a r e d to n o n - d e p r e s s e d d e m e n t e d ~nd n o n - d e m e n t e d d e p r e s s e d p a t i e n t g r o u p s on a f o r c e d - c h o i c e r e c o g n i t i o n m e m o r y task, w h i c h y i e l d s i n d i c e s of d i s c r i m i n a t i o n a b i l i t y a n d r e s p o n s e bias. R e s p o n s e bi a s r e f l e c t s the r e s p o n d e n t ' s t e n d e n c y to s a y "yes" or "ho" w h e n u n c e r t a i n if a g i v e n s t i m u l u s is a t a r g e t or a d i s t r a c t o r item. T h i s s t u d y w i l l a t t e m p t to r e p l i c a t e t h e s e f i n d i n g s a n d n e w l y a d d r e s s th e n a t u r e of d i s c r i m i n a t i o n a b i l i t y a n d r e s p o n s e b i a s in d e m e n t e d p a t i e n t s w i t h d epression. B y i d e n t i f y i n g d i s c r e t e c o g n i t i v e c h a r a c t e r i s t i c s of t h i s d i a g n o s t i c a l l y e n i g m a t i c group, d e m e n t e d p a t i e n t s w i t h d e p r e s s i o n m a y b e m o r e r e a d i l y id e n t i f i e d .

It is e x p e c t e d t h a t t h i s p o p u l a t i o n g r o u p wi l l t r i p l e b y t h e y e a r 2050 d u e to a b u l k of t h e so- c a l l e d " b a b y - b o o m e r " g e n e r a t i o n e n t e r i n g t h e i r s e n i o r years; in addition, i m p r o v e d h e a l t h c a r e is h e l p i n g m a n y

i n d i v i d u a l s in t h i s age g r o u p live l o n g e r lives.

D e m e n t i a is a s y m p t o m of m a n y d i f f e r e n t d i s o r d e r s a n d not a d i s e a s e u n t o itself. C a u s e s of d e m e n t i a i n c l u d e

s truct u r a l , m e t a b o l i c , toxic, a n d p s y c h i a t r i c a b n o r m a l i t i e s . T h e m o s t c o m m o n c a u s e of d e m e n t i a is A l z h e i m e r ' s

d i s e a s e (AD), w h i c h has b e e n f o u n d to a c c o u n t for 50 - 70 % of o l d e r p a t i e n t s w i t h d e m e n t i a seen in p s y c h i a t r i c p r a c t i c e (Kaszniak, 1986). C u m m i n g s a n d B e n s o n (1992) cau t i o n e d , however, t h a t t h e s e f i g u r e s m a y be o v e r e s t i m a t e s ; t h a t is, s i n c e the d i a g n o s i s of A D is o n e of e x c l u s i o n b a s e d on t h e a b s e n c e of o t h e r i d e n t i f i a b l e c a u s e s of d e m e ntia, s u c h a c a t e g o r y is l i k e l y to i n c lude u n r e c o g n i z e d c a u s e s of d e m e n t i a in a d d i t i o n to t r u e c a s e s of AD.

changes, a n d i n t a c t m o t o r f u n c t i o n (Cummings, 198?). A D has a n i n s i d i o u s o n s e t and p r o g r e s s e s gradually, o f t e n leading t o d e a t h in 6-12 years.

As m e n t i o n e d , t h e d i a g n o s i s o f AD is one of exclusion; t h a t is, o t h e r s y s t e m i c or b r a i n d i s e a s e s w h i c h may a c c o u n t fo r t h e c o g n i t i v e d e t e r i o r a t i o n m u s t f i r s t b e r u l e d out. T h e N I N C D S - A D R D A o u t l i n e d c r i t e r i a for possi b l e , probable, a n d d e f i n i t e A D ( McKhann et a l . , 1984). A d i a g n o s i s of d e f i n i t e A D r e q u i r e s h i s t o p a t h o l o g i c a l evidence, o b t a i n e d f r o m b i o p s y or a u topsy, of an e x c e s s n u m b e r of n e u r o f i b r i l l a r y t a n g l e s and s e n i l e p l a q u e s in the c e r e b r a l cortex. T h e h i p p o c a m p u s a n d a m y g d a l a a r e u s u a l l y m o s t s e v e r e l y a f f e c t e d , w i t h m a r k e d c o r t i c a l i n v o l v e m e n t of the f r o n t a l a n d t e m p o r a l l o b e s and l e s s i n v o l v e m e n t of the p a r i e t a l a n d o c c i p i t a l a r e a s (Tomlinson, 1977).

R e d u c t i o n s in t h e n o r a d r e n e r g i c a n d s e r o t o n e r g i c s y s t e m s h a v e also b e e n r e p o r t e d (Bondureff, Mount j o y , and Roth, 1981; Mann,

Lincoln, Yates, Stamp and Toper, 1980; M a n n and Yates, 1983). M u l t i p l e c e r e b r a l i n f a r c t i o n s a r e t h e s e c o n d m o s t c o m m o n c a u s e of d e m e n t i a ( m u l t i - i n f a r c t d e m e n t i a , MID) , a c c o u n t i n g for a p p r o x i m a t e l y 15 - 25% of t h e c a s e s (Kazniak, 1986). M I D is c h a r a c t e r i z e d by an a b r u p t onset, s t e p w i s e d e t e r i o r a t i o n , a f l u c t u a t i n g course, n o c t u r n a l c o n fusion, r e l a t i v e p r e s e r v a t i o n of p e r s o n a l i t y , d e p r e s s i o n , s o m a t i c c o m p l a i n t s , e m o t i o n a l lability, e v i d e n c e of a s s o c i a t e d a t h e r o s c l e r o s i s , a h i s t o r y of h y p e r t e n s i o n , stroke, or t r a n s i e n t n e u r o l o g i c symptoms, and f o c a l n e u r o l o g i c s i g n s on p h y s i c a l e x a m i n a t i o n (Cummings, 1987). T h e c l i n i c a l f e a t u r e s d e p e n d on t h e l o c a t i o n and s i z e of t h e infarcts. D e m e n t i a h a s a l s o be e n a s s o c i a t e d w i t h d e g e n e r a t i v e d i s e a s e s of t h e s u b c o r t i c a l systems, s u c h as P a r k i n s o n ' s d i s e a s e a n d H u n t i n g t o n ' s disease. T h e c o g n i t i v e i m p a i r m e n t s a s s o c i a t e d w i t h s u b c o r t i c a l d y s f u n c t i o n i n c l u d e d e f i c i t s in arousal, a t t ention, m o t i v a t i o n , a n d a s l o w e d r a t e of i n f o r m a t i o n p r o c e s s i n g . M e m o r y d i s t u r b a n c e s i n c l u d e f o r g e t f u l n e s s , c h a r a c t e r i z e d by d i f f i c u l t y s p o n t a n e o u s l y

D e m e n t i a m a y al s o r e s u l t f r o m p o t e n t i a l l y r e v e r s i b l e causes, s u c h a s m e t a b o l i c d e f i c i e n c i e s (e.g., a B12 d e f i c i e n c y ) , t o x i c c o n d i t i o n s (e.g., m i s u s e of m e d i c a t i o n ) , or t r e a t a b l e d i s e a s e s (e.g., p u l m o n a r y or c a r d i a c i n s u f f i c i e n c y , or e n d o c r i n o p a t h y ) (Benson, 1982)* P o t e n t i a l l y r e v e r s i b l e c a u s e s of dementia, w e r e found to a c c o u n t f o r 13,2% of p a t i e n t s p r e s e n t i n g w i t h c o g n i t i v e i m p a i r m e n t (Clairfieid, 1980). A c o m m o n p o t e n t i a l l y r e v e r s i b l e c a u s e of c o g n i t i v e i m p a i r m e n t in t h e e l d e r l y Is an u n d e r l y i n g p s y c h i a t r i c d i s t u r b a n c e , w h i c h is m o s t o f t e n a s e v e r e f o r m of d e p r e s s i o n . C o g n i t i v e i m p a i r m e n t d u e to d e p r e s s i o n has be e n t e r m e d " p s e u d o d e m e n t i a " (Kiloh, 1961), t h e " d e m e n t i a

s y n d r o m e of d e p r e s s i o n " (Folstein and McHugh, 1978), and m o r e r e c e n t l y " d e p r e s s i o n w i t h c o g n i t i v e i m p a i r m e n t " ( F o l s t e i n a n d Rabins, 1992). In s u c h c ases c o g n i t i v e s y m p t o m s r e m i t f o l l o w i n g a p p r o p r i a t e a n t i d e p r e s s a n t t r e a t m e n t . In a r e c e n t s e r i e s of s t u d i e s d e p r e s s i o n was f o u n d t o a c c o u n t for 4.5% of n e a r l y 3000 p a t i e n t s p r e s e n t i n g w i t h c o g n i t i v e d i f f i c u l t i e s (clairfieid, 1988).

A r e v i e w of t h e l i t e r a t u r e by Btoudmire, Hill, Gulley, a n d M o r r i s (1989) i n d i c a t e d t h a t w h i l e t h e m a j o r i t y of

p s y c h o p a t h o l o g i c d i s t u r b a n c e s f o u n d in t h e e l d e r l y (Butler, Lewis, a n d S u nderland, 1991). Da t a r e p o r t e d by the N a t i o n a l I n s t i t u t e of H e a l t h (NIH; 1991) i n d i c a t e d t h a t d e p r e s s i v e s y m p t o m s o c cur in a p p r o x i m a t e l y 15% of c o m m u n i t y r e s i d e n t s o v e r the a g e of 65 a n d a m a j o r d e p r e s s i v e s y n d r o m e o c c u r s In a p p r o x i m a t e l y 3%. In addition, t h e r a t e of c o m p l e t e d

s u i c i d e s f o r o l d e r p e o p l e is h i g h e r t h a n t h a t of the g e n e r a l p o p u l a t i o n . For example, in 1988 t h e s u i c i d e r a t e in the g e n e r a l p o p u l a t i o n of t h e U n i t e d s t a t e s w a s 1 2 . 4/100,000, w h i l e t h e r a t e in t h e 30-84 y e a r old a g e g r o u p was

D i a g n o s i s of d e p r e s s i o n in t h e e l d e r l y c a n be a

S l e e p and a p p e t i t e c h a n g e s o c c u r in nor m a l a g i n g (Butler e t al., 1991; Dement, Miles, and carskadon, 1982) a n d f a t i g u e m a y be r e l a t e d to h e a l t h p r o b l e m s as o ften as t o d e p r e s s i o n (Salzman a n d Shader, 1979). S o m a t i c s y m p t o m s in the e l d e r l y s h o u l d be a t t r i b u t e d to d e p r e s s i o n o n l y a f t e r a t h o r o u g h p h y s i c a l e v a l u a t i o n (Gallagher, 1986). D i a g n o s i s and t r e a t m e n t of l a t e - l i f e d e p r e s s i o n is c o m p l i c a t e d f u r t h e r b y t h e f r e q u e n t l y o c c u r r i n g admixture, overlap, and i n t e r a c t i o n o f c o g n i t i v e d e f i c i t s and

d e p r e s s i v e s y m p t o m a t o l o g y . S o m e a u t h o r s h a v e n o t e d th a t d e p r e s s e d patients' c o m p l a i n t s a b o u t i m p a i r e d c o g n i t i v e f u n c t ioning, p a r t i c u l a r l y memory, t e n d to o v e r e s t i m a t e t h e e x t e n t of t h e i r a c t u a l diffi c u l t y , as m e a s u r e d by

p s y c h o m e t r i c t e s t s (Kahn, Zarit, H i lbert, N i e d e r e h e , 1975). It has b e e n sug g e s t e d , however, t h a t w h i l e s u c h d i s p a r i t y m a y e x i s t in m i l d l y o r m o d e r a t e l y d e p r e s s e d p a t i e n t s , s e v e r e l y d e p r e s s e d p a t i e n t s a r e m o r e l i k e l y to e x p e r i e n c e s i g n i f i c a n t c o g n i t i v e d i f f i c u l t i e s (Raskin, 1986). Henry, W e i n g a r t n e r , a n d M u r p h y (1973) f o u n d t h a t t h e i n t e n s i t y of d e p r e s s i o n w a s i n v e r s e l y c o r r e l a t e d w i t h h o w m u c h c o u l d be l e a r n e d and r e m e m b e r e d . A s m e n t i o n e d p r e v i o u s l y , a p r e d o m i n a n t l y e l d e r l y s u b g r o u p of d e p r e s s e d p a t i e n t s are e s p e c i a l l y v u l n e r a b l e to d e p r e s s i o n - r e l a t e d c o g n i t i v e d y s f u n c t i o n , c h a r a c t e r i z e d m o s t o f t e n b y d e f i c i t s in

s t a t e s e r v e s to bi a s c o g n i t i v e p r o c e s s i n g . F o r example, Be c k (1972) a r g u e d t h a t d y s f u n c t i o n a l c o g n i t i o n s d e f i n e the

d e p r e s s e d state. T h a t is, in d e p r e s s i o n a n e g a t i v e v i e w of t h e self, world, and future, l o w e r e d s e l f - e s t e e m , and

d e c r e a s e d m o t i v a t i o n s e r v e as a c o g n i t i v e f i l t e r t h r o u g h w h i c h all o t h e r e x p e r i e n c e s are p e r c e i v e d . B e c k ' s t h e o r y of d e p r e s s i o n was r e f o r m u l a t e d in t h e " l e a r n e d h e l p l e s s n e s s " m o d e l (Maier a n d Seligman, 1976) . Th i s m o d e l c o n t e n d s th a t d e p r e s s e d p a t i e n t s e x p e c t u n f a v o r a b l e e v e n t s t o occur, feel t h e y h a v e n o c o n t r o l o v e r t h e s e n e g a t i v e c o n s e q u e n c e s , and as a r e s u l t d e m o n s t r a t e p a s s i v i t y and a n e g a t i v e c o g n i t i v e set. It h a s b e e n a r g u e d t h a t s u c h a n e g a t i v e c o g n i t i v e sot a f f e c t s n o t o n l y o n e ' s p e r c e p t i o n of o n g o i n g events, b u t a l s o a c c e s s i b i l t y a n d r e t r i e v a l of i n f o r m a t i o n a l r e a d y in m e m o r y (Weingartner, Miller, and Murphy, 1977). For example,
