“The report indicated that as many as 44.000 to 98.000
people die in hospitals each year as the result of medical
errors.
Even using the lower estimate, this would make medical
errors the eighth leading cause of death in this country –
higher than motor vehicle accidents (43.458), breast cancer
(42.297), or AIDS (16.516). About 7.000 people per year
are estimated to die from medication errors alone – about 16
percent more deaths than the number attributable to
work-related injuries.”

“The correct action was taken 99,0 percent of the time,
translating to 1,7 errors per day. One out of five of these
errors was serious and/or potentially fatal. If performance
levels even substantially better than those found in the ICU
(for example 99.9%, a 10-fold reduction in errors) were
applied to the airline and banking industries, it would still
equate to two dangerous landings per day at O’Hare
International Airport and 32.000 checks deducted from the

About a third of patients rated as "appropriate" candidates for
percutaneous transluminal coronary angioplasty (PTCA) or
coronary-artery bypass grafting (CABG) instead were treated medically, and
these medically treated patients had worse outcomes than the
patients treated in accordance with the appropriateness criteria.
As compared with patients who had indications for which CABG was
rated appropriate and who received the procedure, patients with
indications for which CABG was considered appropriate and who
were instead treated medically had four times the risk of death or
nonfatal myocardial infarction and three times the risk of angina one
year later. Similarly, the patients who had indications for which PTCA
was rated appropriate but who received medical treatment had a risk
of angina one year later that was about twice that of patients who had
indications for which PTCA was rated appropriate and who received
PTCA.
