The relationship between external factors -such as plaque ecolo­

gy, sali va, oral hyg iene, di etary habi ts, soci al and cul tural circumstances- on the one hand and enamel de- and reminerali z a­

tion on the other i s schematically given in fi gure 7.1 .

The results of a large scale epi demiologi cal experiment publi shed i n 1975, showed that substitution of sugar in the dai ly diet by other sweeteners, such as xyli tol lead to 10 0 % caries reducti on.

Thi s finding can be explained by the effect of xylitol ei ther (i ) on plaque,

Chapter 2 g i ves a review of the literature on the effects of xy­

l i tol in epi demiologi cal studies, and studi es on plaque, sali va and enamel deminerali zati on. Most epi demi ologi cal studi es showed that xylitol had substanti al cari es reduci ng properties. The studi es on plaque showed that xyli tol reduces growth and aci d producti on of some, but not all, mi cro-organi sms. Xylitol also seems to reduce the thi ckness of the plaque layer on the teeth and appears to affect the calci um content i n plaque coveri ng the teeth. The studies on saliva show a reducti on i n the number of acid produci ng mi cro-organi sms under the i nfluence of xyli tol.

Several studi es on enamel de- and reminerali z ati on, usi ng an i n vivo model, have shown mineral gain after dai ly appli cati on of a xylitol soluti on on enamel. Comparable resul ts were found in la­

boratory studi es.

The main conclusions of this in vitro e xperime nt are :

- A l inear re lation was foun d betwe e n the l e ngth of the d e mine r­

alization pe riod and the l esion d e pth al ong the walls and bottom of the grooves. This shows that the ions pre sent at the surface of the mine ral cry stallite s in the e name l d etermin e l e sion pro­

gre ss.

- L e sion d e pth in the bottom of a groeve was approximate l y 2 X the le sion d e pth at the wall of a groeve. No re lation was found betwe e n le sion d e pth and d e min era lization period i n the l e sions in the surface ename l .

- Enamel d e mine ralizati on in the groove s took place without the formation of a surface l ay er. In contrast, d e mineralization of surface e name l alway s resulte d in the formation of subsurface le sions with a surface lay er.

Onder zoek van carië s of tandbederf levert informatie op omtrent de behandelmogelijkheden en preventie van deze mondziekte. De ziekte bestaat in feite uit mineraalver lies uit het glazuur - de harde toplaag- en het dentine - het iets minder harde tandbeen­

van de gebitselernenten. Dit mineraalverlies resulteert uitein­

delijk in c aviteiten ( gaten) in tanden en kiezen , pijn , verl.ies van kauwvermogen en verkleur ingen van de tanden. Voordat deze caviteiten met het blote oog goed zic htbaar zijn , is er al mine­

raal ver dwenen. H et beginstadium van tandbederf , de "initië le lesie" , is op zic hz elf een onschuldige afwijking van de buitenste laag glazuur van tanden en kiezen.

Afhankelijk van de omstandigheden in de mond kan een initië le lesie:

( 1 ) weer genezen door afzetting van c alc ium en fosfaten in de le­

sie vanuit tandplaque of speeksel. Dit pr oces wordt reminerali­

sat i e genoemd.

( 2) verder ontkalken door verlies van meer c alcium en fosfaat vanuit het g l azuur naar tandplaque of speeksel. Dit proces wordt demineralisatie genoemd.

In werkelijkheid wisselen per iodes van reminer alisatie en demine­

r alisatie elkaar frequent meerdere malen per dag af. Het netto resultaat van de- en remineralisatie bepaalt of er in het glazuur sprake is van respectievelijk mineraalverlies of mineraalwinst.

Van pr imair belang voor de- en remineralisatie is de concentratie van de versc hillende ionen , zoals c alc ium , fosfaat en fluoride en de zuurgraad bij het tandoppervlak. Aller lei factoren beï nvloe­

den de concentratie van deze ionen en daarmee dus ook het tandca­

rië sproces. Der gelijke factoren zijn bijvoorbeeld de samenstel­

ling en hoeveelheid tandplaque en speeksel , de bacter ië le mond­

flora , het gebruik van de tandenbor stel , eetgewoonten, enz.

S ame nvattin g

In 197 5 werde n de resultaten gepublicee r d van ee n g r oot car ië son­

dcrzoek in Finland, waarin p r oe fpe r sone n drie j aar lang totaal gee n suike r gebr uikte n. In plaats daarvan we r de n voedse l e n d ranke n gezoet met de alte r natie ve zoetstof xylito l . Na deze d r ie j aar kon bij deze g r oep gee n toe name van carië s wor de n ge me ­ te n, te r wij l nor maal suike r gebruik in de controle g r oe p we l le idde tot ee n car ië stoe name.

D i t resultaat kon wor de n ve rklaard door effecte n van xylitol op ( i) tandplaque, ( ii) speeksel, ( iii) het de - e n remine r alisatie ­ p r oces, of door combinaties van deze e ffecte n.

Het doel van het in dit pr oefschrift beschreve n onde r zoek is het bepale n van de

tandglazuur in

invloed van xylitol op de - e n re mine r alisatie van de mond.

In hoofdstuk 2 wor dt ee n ove rzicht ge geve n van de e ffecte n van xylitol , zoals die sinds 197 0 in de vaklite ratuur zij n beschre­

ve n. E r we r d onde r scheid ge maakt in

- e p i de miolog i sch onde r z oek e n dierexpe r i me nte n , - tandplaque onde r zoek,

- speekse l onde r zoek e n

- onde r zoek van glazuur de- e n r cmine r alisatic.

De mee ste e pidemiologische e n die r -expe r ime nte n geve n ee n aan­

zie nlij k car ië sre mme nd effect aan van xylitol.

Tandplaque e xpe rime nte n ve rtone n r e mming van g r oe i e n zuurpro­

ductie van sommige bacte r ië n door xylitol. Ook lij kt de dikte van de plaque laag op tande n e n kieze n af te neme n e n de calcium­

conce ntratie in plaque te ve r ande re n.

Bij speeksel onde r zoek we r d ee n afname gevonde n van het aantal zuurpr oduce r e nde bacte r ië n na gebr uik van xylitol bevatte nde voe ­ dingsmidde le n.

E xpe r ime nte n waar bij het re- e n demine r alisatiepr oces van glazuur we r d onde r zocht , ge ve n aan dat dagelij kse applicatie van ee n wa­

te rige xylitoloplossing op glazuur resulteert in remincr alisatie.

In genoemde e xperimenten werd xy l itol op ver sch i l l en de manieren aan proe fpersonen en proefdie re n aangeboden. Soms werd al le ge­

wone suiker ( sucrose) in het dage l i jkse dieet vervangen door xy ­ litol, in andere gevall en werd xy litol alleen gebru i kt in de vorm van xyl i tol bevattende snoepjes, chocola, koekje s of kauwgum.

Deze manieren van toedienen. maken het echter onmogel ijk vast te stel len of de gevonden car iësreducti e nu wordt veroorzaakt door de invloed van xylitol op plaque, speeksel of direct op het r e­

en demineralisati eproces.

In hoofds tuk 3 t ot en me t 6 worden twee kl inische e xperime n ten be schreven waarin is gepoogd het effect te bepalen van xylitol op re- en demineral i sat ie, z orider toevoeging van xyli tol aan de dagel i jkse voeding.

l n de hoofdstukken 3 en 4 wordt de .invloed beschreven van xy l i tol bevatten de tandpast a ' s op tandglazuur in e en re mineral isatie be-vorderende situatie. Hiervoor werden stee ds twee blokjes g l az uur gemonteerd in de opstaande wand aan de wan gzijde van een ge bits-pr othese van een bits-proefpersoon. Vooraf waren kunstmatig z eer lichte ontkalkingen in het gl azuur .aangebracht, tot een diepte van ongeveer 0 , 02 mm. Gedurende 4 weken poetsten 20 proefper­

sonen twee maal daags de g l azuurbl okj es buiten de mond met een ex-perimente le tandpasta. Daarna werden de blokjes u i t de prothe se genomen e n twee nieuwe blokjes gep l aatst, waarn a gedurende op­

nieuw 4 weken een andere tandpasta werd gebruikt. Z o werden in totaal door i edere proefpersoon de vol gende 4 tandpasta ' s ge­

bruikt:

( l ) pasta X op basis van xylitol, ( 2 ) pasta G op basi s van gl ycerol,

( 3 ) pasta XF op basis van xylitol met fl uoridetoe voeging en

( 4 ) pasta GF op basis van glycerol met fluoride toevoeg i ng.

Me t behulp van micro-hardhei dsmetingen van het gl azuurop pervlak werd een i ndruk verkregen van re- e n de mineral i satie van deze

blokjes in de mond.

Het blij kt dat do glazuurontkalkin ge n in 4 weke n duide lijk harde r worde n ( re mineral i sere n ) , maar dat de aanwe zighe id van xylitol of fluoride in de tandpasta ' s hie rop gee n meetbare invloed heeft.

In hoofdstuk 4 wordt beschreve n hoeveel fluoride geduren de de vier weke n die de proef duurde , in de glazuurontkalkinge n is op­

ge nomen. H iervoor werd de fluoride conce ntratie in het glazuur bepaald me t be hulp van twee ve rschille nde meette chn ieke n -een me­

chan ische schuurtechniek e n een ge avanceerde electronische tech­

n iek- aan ho t e inde van de testperiode s.

Me t de schuurtechniek werde n 5 zeer dunne l age n glazuur ( dikte ± 0 , 0 0 7 mm) van het oppervlak van de bl okjes verwij de rd en che misch geanalyseerd. In de buite nste glazuurlaag bleek ongeveer 2 0 0 ppm fluoride aanwe zig te zijn, in de die pe re lage n onge veer 1 0 0 ppm.

Met behulp van Secondary Ion Mass Spectrometry ( S IMS ) werd, e venals in het voorgaande e xpe rime n t, alleen in de buite nste gla­

zuurlaag een verhoogde fluorideconcentrat.ie aan getroffe n C ± 500 ppm fluoride op minder dan 0 , 0 07 mm vanaf het oppervlak) .

Er werde n gee n statistisch sign ificante ve rschille n gevonde n tus­

se n de fluoride opname in glazuur voor de verschille nde tandpas­

ta ' s. Er be stond echte r we l ee n tre n d dat de be ide xylitol bevat­

te n de tandpasta ' s een hogere fluoride conce ntratie in glazuur op­

l e verde n dan de glycerol bevatte n de pasta ' s.

In de hoofdstukke n 5 e n 6 wordt het tweede klin ische e xperime nt be schre ve n . H ierin wordt he t effect be paald van waterige xyl i-toloplossin ge n op tandglazuur in een demine ralisatie bevorde re n de situatie . ( He t eerste e xperime nt we rd immers gedaan in een remi-ne ralisatie bevorde re n de situatie . )

Ook nu werde n wee r glazuurblokjes ge monteerd in de onderprothe se s van een aantal vrij wil ligers. De belan grijkste verschille n me t he t ee rste e xpe rime nt zijn :

derzocht op welke wijze onder laborat oriumomstandigheden demine­

ral isatie van deze glazuurstukjes plaats vind t. Dit onderzoek voorafgaan d werden kunstmatige glazuurl esies aangebracht t o t een diepte van ± 0 , 045 mm in het glaz uuroppervlak en + 0, 03 en 0, 0 5 mm i n d e wanden en bodem van de gro e fjes.

De vrijwil l igers gebruikten gedurend e 16 dagen een 2, 5% xyl it o l -, een 2 , 5% suiker- of een wa ter opl ossing. De pro these met de

b l okjes werd t w ee keer per dag gedurende 5 minuten in de t es t op­

l o s sing gedompe l d . Na 16 dagen werden de bl okj es gl azuur uit de pro these v erwij derd en t w ee nieuwe gem o n teerd v o or he t gebruik v an de v ol gende tes t opl o s sing . Na opnieuw 16 dagen werd het wis ­ s el en h erhaal d z oda t uiteindel ijk e lke v rij wil l iger a l l e drie t es t opl o s singen had gebruik t . Hierna werden de lesiediepte en he t mineraal v er l ies in de g l a z uurbl ok j e s verder onder z o cht me t behul p van po l arisat iemicro s c opie en mic r oradiografie .

H e t blijkt dat he t gl az uur aan he t o ppervlak en in de groeven v erder deminera liseert tijdens de t e s t periodes. In de groeven werd geen vers c hi l gev onden t u s s en de behandelingen me t xylit ol, s uiker en w a t er. Het gl azuur van he t oppervlak v an de bl okj e s v er t o onde een duidelijk ander re sul t aa t . Hier werd een be l ang­

rij ke remming gev onden van he t mineraal v erlies do or de xylit olbe­

hande l ing in v ergelij king met z o we l de suikerbehandel ing a l s de w a terbehande l ing.
